At PT Instant, we are considered an out-of-network provider, which gives us the flexibility to focus on personalized, one-on-one care—without the limitations that insurance networks often impose.

But here’s the good news: many insurance plans, especially PPOs, offer out-of-network benefits that can reimburse you for a significant portion of your visit costs.

Which Insurance Plans May Cover Out-of-Network PT?

If you have one of the following plan types, there’s a good chance you’re eligible for reimbursement:

  • PPO (Preferred Provider Organization) – Offers the most flexibility and typically covers a portion of out-of-network services after a deductible is met.

  • POS (Point of Service) – May offer some out-of-network benefits with a referral from your primary care provider.

  • Certain Medicare Advantage PPO plans – These may offer out-of-network benefits, but it's best to confirm directly with your plan.

On the other hand, HMO plans and traditional Medicare or Medicaid typically do not cover out-of-network services unless it's an emergency.

  2. We provide you with a superbill—a detailed receipt that includes necessary medical codes.

  3. You submit the superbill to your insurance company using their out-of-network claim form.

  4. Your insurer may reimburse you directly for a portion of the visit cost, depending on your plan’s out-of-network coverage.
